I understand that any misrepresentation or material omission made by me on this application will be sufficient cause for cancellation of this application or immediate termination of employment if I am employed, whenever it may be discovered.
If I am employed, I acknowledge that there is no specified length of employment, and that this application does not constitute an agreement or contract for employment. Accordingly, either I or the employer can terminate the relationship at will, with or without cause, at any time, so long as there is no violation or applicable state or federal laws.

I also understand that if I am employed, I will be required to provide satisfactory proof of identity and legal work authorization within three days of being hired. Failure to submit such proof within the required time shall result in the immediate termination of employment.
I understand and accept that Mainsail is a drug-free workplace, and that Mainsail maintains a policy of “Reasonable-Suspicion Drug Testing.” If at any time it believes that an employee is using or has used drugs in violation of its drug-free workplace policy based on specific objective and articulable facts and reasonable inferences, Mainsail may require that the employee undergo drug testing. Such inferences at work may be based on (1) Observable phenomena, (2) Abnormal conduct or erratic behavior; (3) A report of drug use provided by a reliable and credible source; (4) Evidence that an employee has tampered with a drug test during their employment with Mainsail; (5) Evidence that an employee has caused, contributed to, or been involved in an accident while at work; or (6) Evidence that an employee has used, possessed, sold, solicited or transferred drugs while working, or while operating machinery or a vehicle while on Mainsail premises. (F.S. §440.102(1)(n); (5)2(o).
Similarly, if I am injured in the course and scope of my employment, I must submit to a drug screen test.>

Mainsail Lodging & Development (The Company) may obtain a consumer report(s) (also known as a background check report) about you from a consumer reporting agency for employment purposes, including as an applicant for employment or from time to time during your employment.

I understand that, as allowed by applicable law, the Company may rely on this authorization to order additional background check reports, including investigative consumer reports and any consumer credit reports* (1) during my employment or time as a volunteer or independent contractor, as applicable, and (2) from any CRA other than ADP SASS without asking me for my authorization again. I understand the Company may order background check report(s) under my legal name and any other names I may have used.
I also instruct and authorize the following persons, agencies, and entities to disclose to ADP SASS and its agents all information about or concerning me, as allowed by law, including but not limited to: my past or present employers; learning institutions, including colleges and universities; law enforcement and all other federal, state and local agencies; federal, state and local courts; the military; credit bureaus; testing facilities; motor vehicle records agencies; all other private and public sector repositories of information; and any other person, organization, or agency with any information about or concerning me. As allowed by law, such disclosures may contain the following information pertaining to me: credit history*; public records; a Social Security number verification; driving records; military service; credentials/certifications; worker’s compensation injuries; and verification of prior employment and education.
